When checking in at SMC, whether being admitted or visiting ED or Outpatient services for tests or a simple procedure, be sure to get registered for the hospital’s Patient Portal.
“During registration, give your email address or mobile phone number to the registrar or ask a nurse at any time during your stay to begin the enrollment process. From there, it is very easy to sign up,” said Clinical Nurse Informatics Liaison Molly Netto. “YourCareEverywhere is a secure Patient Portal and health and wellness website that lets you access and interact with the health information you care about most.”
The staff member will send the patient a link via email or text. That link is used to register for the Patient Portal. Once registered, the patient will be notified of lab, radiology, and test results, and other important information about medical records.
Patients who use any of our SMC Health Physicians practices also have access to a Patient Portal at portal.cybermedehr.com. This Patient Portal allows patients to request appointments with any SMC Health Physician, view medical records, and have access to general health and wellness articles.
