When a patient is ready to leave the hospital, but still needs intermittent care, Salem Home Health steps in.
Salem Home Health provides community-based healthcare and support services for patients with an acute illness, long-term medical condition or permanent disability.
“Our team consists of skilled nurses, speech therapists, physical therapists, medical social workers, occupational therapists and certified home health aides,” said Home Health/Hospice Administrator Kim Willis, M. Sc. A. “The team improves the overall health of residents in the Salem County area by providing home-based, comprehensive, quality care and service in a cost-effective and compassionate manner. We operate with the motto, ‘Let our family take care of yours.’”
Home Health works closely with the patient’s primary care physician to provide a care plan customized to the patient’s specific needs.
“Our goal is to get our patients back to performing daily activities so they can care for themselves,” said Willis. “These dedicated workers deserve our recognition and praise. And a special thank you to our HHAs during Home Care Aide Week. It takes a special person to work in Home Care.”
