SMC offers the Pfizer COVID-19 vaccine for children and teens age 12 to 17. Moderna vaccines — as well as Pfizer — are available for adults 18 and older.

Vaccines are administered:

  • Friday: 8 to 10 a.m. and 3 to 5 p.m. Moderna for residents 18 and older
  • Saturday: 8 to 11 a.m. Pfizer for residents 12 and older

Minors must have consent from a parent or guardian to receive the vaccine. Consent can be written or verbal. Print out the consent form and bring a signed copy to the facility or arrange for verbal consent.

An appointment is required to receive the vaccine. To make an appointment, call 856-469-8555 and leave a message. Someone will return your call to schedule an appointment.

Vaccines are administered in the Outpatient Registration area off the main lobby. Please check-in at the front desk.
